OK529 Payroll Deduction Form
Used to establish a Payroll Deduction Plan on an existing OklahomaDream 529 Plan account. Form can also be used to stop payroll deductions and change the allocations on your Payroll Deduction Plan

Account Application
Use this fillable form to open an account in the OklahomaDream 529 Plan.

Rollover Form
Use this form to rollover assets to the OklahomaDream 529 Plan from either: another Section 529 College Savings Account, a Coverdell Education Savings Account (formerly an Education IRA), or redeemed qualified U.S. Savings Bonds (see Plan Disclosure Statement for additional details).

Additional Investment Form
Use this form to make additional contributions to an existing account in the OklahomaDream 529 Plan. This form cannot be used to establish a new account.

Change Form
Use this form to make changes to an existing OklahomaDream 529 Plan Account. Types of changes permitted include an Account Owner Change, Bank Account Information Change, Change of Beneficiary, Investment Option Change, Mailing Address Change and more.

Withdrawal Request Form
Use this form to request a full or partial withdrawal from your OklahomaDream 529 Plan Account.

Withdrawals from a 529 Plan for qualified expenses are free from federal income tax. Qualified expenses include tuition and fees, room and board, books and other supplies. Non-qualified withdrawals are subject to federal income tax and a 10% penalty tax on earnings, with some exceptions, and may also be subject to state tax.
